If you need us before or after our regular office hours due to an emergency situation, please call 911. If you need us for a non-urgent matter that does require after hours attention, please call our office at (720) 475-8750 and be connected to the on call physician.

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (720) 475-8750 during regular business hours.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

During Your Visit

When you check in, you will be asked to provide your completed paperwork, a photo ID, your medical insurance card and pay for any co-payment required by your insurance. Your record will be prepared while you wait for your scheduled appointment time.

A medical assistant will bring you back to an exam room and have you change into a pair of our shorts. She will take your vitals, take pictures of your legs, and ask you a series of questions related to your symptoms, medications and medical history.

The provider will look at your legs, and review your current and past medical conditions and concerns. She will then order an ultrasound of your leg(s). We have 2 providers on staff who see new patients. Dr. Hammond and Mandi Phillips (our Nurse Practitioner) see patients on Wednesdays. The initial appointment will be the same regardless of who you see. If you see Mandi and she determines that you are a candidate for the minimally invasive closure procedure, you will also meet with Dr. Hammond for a short consultation.

Jody, our Registered Vascular Technologist, will bring you from the exam room to the ultrasound room and perform the ultrasound.

After the ultrasound, you will get dressed and move to the consultation room. You will watch a short (2-3 minute) film on the Venefit (VNUS closure) procedure, then the provider will visit with you regarding their recommendations and answer any further questions that you may have.

Expect to spend 1.5-2 hours in our office for your new patient appointment. Please plan accordingly with your parking.

Check-out. Your visit will be billed to your insurance. If applicable, we will collect a co-pay for the specialist consultation and for the ultrasound. You may be billed for charges as outlined in your insurance contract (i.e. deductible, co-pay, co-insurance and other applicable charges.)

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

All self pay patient accounts, excluding elective cosmetic procedures, facility designated self-pay flat rate procedures and scheduled/discounted procedures for international patients will be given an uninsured discount of 35%.

Working With Insurance

We are a participating provider with most insurance carriers. Please check with your particular insurance to verify you have the necessary coverage. We also offer self-pay packages. To find out more, please contact our office. We’d be happy to answer any questions you have about how insurance contributes to vascular surgery and vein clinic costs.

While we take most insurance policies, we do not accept Kaiser Insurance, which is a statewide exception. Our insurance statement and policy can be found in the SVC Welcome Letter and Financial Release documents.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
